Upon further review, since the back of the neck is painted white, it is a National Glenwood model 99 guitar (same years).

The definitive answer is that this is a National Glenwood guitar, made from 1962-65. If it has a piezo pickup in the bridge, it is a model 98, otherwise it is a model 95. It is a semi-hollow 'res-o-glas' (fiberglass) guitar made by Valco, which also made similar guitars sold under their own National and Supro brands, as well as those sold by Montgomery Ward under the Airline brand (aka the JB Hutto model used by Jack White). Eastwood currently makes a similar map guitar but it is chambered wood.
